The Nurse I works under the direct supervision of the Nurse Manager to provide routine care and treatment through the application of the nursing process. Is responsible for maintaining nursing standards in accordance with the Nurse Practice Act, HHSC Texas Administrative Codes and other facility policies, procedures and standards. Communicates and collaborates with other shift RN's to ensure 24-hour continuity of care. Ensures adequate staffing according to characteristics of the patient population; determines staff assignments based of appropriate knowledge and skills to meet individual patient needs. Models and supports a proactive approach to unit management using the principles of Social Learning, and demonstrates a commitment to a culture that minimizes the use of restrictive interventions. Serves as an effective member of the treatment team to facilitate a multidisciplinary/holistic approach to care. Serves as a preceptor for students and newly hired RN's with a focus on recruitment and retention. Provides direct supervision to all Services Assistant Staff and Licensed Vocational Nurses. Participate in the screening/interviewing process for prospective unit staff, oversees unit orientation for all new employees, and ensures on-going training and staff competence. Ensures position descriptions and performance evaluations are presented within established time frames. May receive collateral, consultative coaching and/or training from Social Learning implementation leaders or consultants.

Must understand that duties, location of work and shift patterns are subject to change, as determined by the administration, to meet the needs of the hospital.

The Clinical RN has no financial responsibility but is accountable for assigned property and appropriate allocation of human resources.

Essential Job Functions:
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.

1. Performs comprehensive assessment of patients' nursing care needs including physical, behavioral, psychiatric, and social needs.
2. Observes and analyzes patient behavior and status and documents in the medical record and reports as needed to other team members, including the attending physician.
3. Develops or contributes to the development of the individualized treatment plan in collaboration with other treatment team members and based on data obtained from the assessment.
4. Provides comprehensive individual or group health teaching and coping strategies to patients and/or families in order to meet patient/family needs.
5. Utilizes one-to-one counseling skills in therapeutic interactions with patients to assist them in identifying anxiety and developing coping strategies.
6. Initiates and implements population-specific nursing interventions based on appropriate standards of care for the assigned patient population.
7. Administers medications and treatments as ordered by the physician and maintains medication administration (MAR) and controlled drug records according to facility policy.
8. Establishes and monitors the therapeutic milieu through clinical supervision of LVNs, and Psychiatric Nursing Assistants (PNAs).
9. Provides training to subordinate staff in psychiatric nursing principles, unit routines, and milieu management.
10. Monitors Treatment Intervention and Protection Strategies (TIPS) and in the absence of other staff members implements TIPS.
11. Initiates and monitors the appropriate use of restraint or seclusion to ensure the safety of the patient and staff members.

Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.


Knowledge Skills Abilities:
1. Comprehensive knowledge of nursing procedures and practices.
2. Comprehensive knowledge of the Texas Nursing Practice Act, Rules and Regulations of the Board of Nurse Examiner's for the State of Texas.
3. Ability to provide a competent level of specialized, population-specific nursing care to the assigned patient population (i.e. Psychiatric/Mental-Health, Medical-Surgical, Child-Adolescent, etc.)
4. Ability to perform a comprehensive nursing assessment inclusive of the patient's mental health, medical, social, and psychological needs.
5. Ability to identify nursing interventions appropriate to the population served.
6. Ability to formulate population specific nursing diagnoses or problem list from the assessment of the patients nursing care needs.
7. Ability to manage and take a leadership role in managing emergencies.
8. Skill in communicating effectively with other staff members in order to provide effective clinical supervision or to relay concerns about the patient to other team members.
9. Ability to obtain certification for basic Cardio-Pulmonary resuscitation CPR) and ability to perform CPR in the clinical setting.
10. Ability to complete Treatment Intervention and Protection Strategies (TIPS) prior to work on patient care unit.
11. Ability to teach others the principles of psychiatric nursing and maintenance of the therapeutic milieu.

Registration or Licensure Requirements:
1. Licensed as a Registered Nurse to practice professional nursing in the State of Texas or a state that recognizes reciprocity through the Nurse Licensure Compact.
2. Must possess valid Texas driver's license or obtain it no later than 90 days after hire date. Applicants with an out-of-state driver's license must provide an original certified driving record from the state of driving licensure. Eligible driving record required based on HHSC Fleet Management policy.

Waco Center for Youth is a psychiatric residential treatment facility that serves teen-agers, ages 13 through 17, with emotional difficulties and/or behavioral problems.
